Notary Certificate – Type 3

The third type of notary certificate is used in place of the first two types of notary certificates. This option is written directly into the acknowledgement or jurat and does not require a separate notary seal. Using this option means that you will only need to make one impression of your notary seal and sign one time.

The information required to be contained in this type of notary certificate depends on if you are replacing the notary certificate type 1 or type 2. If you are replacing the notary certificate type 1, then you would need to include all the required items of the notary certificate type 1. If you are replacing the notary certificate type 2, then you would need to include all the required items for the notary certificate type 2. Again, if you are unsure if the document is being recorded at the Bureau of Conveyances, it is a good idea to go with the type 1 notary certificate.

Below are blank and filled examples of the third type of notary certificate
