Signature By Notary

There are times that a person appearing before a notary is physically unable to sign a document with a signature or mark due to illness, quarantine, disability, or other extenuating circumstance. In this situation a notary may, with the consent of the signer, sign the name of the signer and write “Signature affixed by notary pursuant to section 456-19, Hawaii Revised Statutes” next to the signature on the document and in the notary journal. In some cases, two impartial witnesses may be required for a signature by notary. The notary should record the witnesses and signatures in the notary journal.

In this example the notary is signing on behalf of John Smith Doe.
